About the RoleWe are seeking a highly skilled Registered Nurse to join our Patient Transfer and Access Center (PTAC) team as a Case Manager. This is an exciting opportunity to work in a dynamic and fast-paced environment, collaborating with a multidisciplinary team to optimize patient flow and capacity management across our system.Key Responsibilities

• Develop and implement effective case management strategies to ensure timely and efficient patient flow.
• Collaborate with hospital-based case managers, capacity leaders, and other stakeholders to identify and address capacity challenges.
• Manage and coordinate repatriation programs to optimize bed utilization and reduce unnecessary transfers.
• Provide expert guidance and support to hospital-based case managers and other team members on case management best practices.
• Participate in system-wide initiatives to improve patient flow, capacity management, and quality of care.Requirements
  • Graduate of an approved school of nursing, BSN required.
  • Current license to practice as a registered nurse in the State of Massachusetts.
  • Case Manager Certification preferred.
