Everything you and your family need, right in your neighborhood to be your best healthy.Job DescriptionProvides evening and weekend social work and case management services to patientson the inpatient Geriatric Med-Psych unit, based on local and national Professional Social Work Standards and Code of Ethics.Completes psychosocial assessment on assigned inpatients within 72 hours of admission.Assesses discharge planning needs for assigned patients and implements completed aftercare plan.Maintains contact with patient's family, other mental health providers, and other collaterals in order to facilitate their input and involvement in treatment and discharge planning.Conducts utilization review as requested or required on all assigned patients when patient insurance plan requires. Facilitates group treatment in the milieu.
