Licensed Practical Nurse (LPN)EXCELLENT SHIFT & WEEKEND DIFFERENTIALS, BENEFITS, AND MUCH MORE!!*** SUB-ACUTE SHIFT BONUS & SIGN-ON BONUS $3000 (FT 30 HRS OR MORE) OR $1500 (PT 20-30 HRS)***Job Responsibilities of the Licensed Practical Nurse (LPN)Ability to assess the healthcare needs of the patient/resident, make a nursing diagnosis, transition this through collaboration with the nursing team, into an interdisciplinary plan of care.Is able to communicate this plan to all care providers in a way that ensures continuity of care for the patient/resident.Demonstrates the ability to establish and maintain functional communications with staff, other disciplines, families, medical staff, and communityDemonstrates the proficiency with computerized documentation.Demonstrates responsibility for one's own practice, utilizing educational opportunities to expand or refresh skills and/or knowledge and then demonstrates the ability to apply knowledge and share with others.Demonstrates the ability to provide education to patients/residents regarding their healthcare issues and care plan strategies.
