Pay: $3,378 to $3,498 per week Shift Information: Days - 5 days x 8 hours Start Date: ASAP About the Position Facility: Home Health & Hospice EPIC EMR PROJECT. Must be 1/12 start, 12 weeks. Medical Social Worker for Home Health and Hospice. Will be all 8 hour days but may also be expected to cross cover adult home health, hospice and community hospice as needed.Requirements: Education: Master's Degree in Social Work (MSW), Experience: 2 years of relevant experience, Licensure: Vermont LMSW required. EMR is netsmart, converting to Epic. Epic experience is required in the past 24 months.
