Experience: Minimum of 1 year of home care or hospice experience
License: Must be licensed and fingerprinted in the state
Certifications: BLS Certification, Valid Driver's License, and
Insurance Must-Have: 1 year of home care or hospice experience,
preferred experience with Meditech home care EMR, reliable car, and
auto insurance Description: Seeking a Hospice Registered Nurse to
provide care to adult and geriatric patients. The nurse will be
responsible for medication management and treatment plans for
patients with various diagnoses including COPD, post-surgical care,
orthopedic, CVA, dementia, and hospice. Evaluations and development
of care plans will be conducted regularly. Visits will typically be
within a 20-minute radius in an urban area. Average case load is 5
to 6 visits per day, with a point system in place for scheduling.
Weekend call is required, with a response time to clients within 20
minutes. Self-scheduling is permitted during weekdays, with a
rotation for weekend coverage. The dress code is flexible, provided
attire is clean and professional. A week of orientation with a
preceptor will be included. The role includes working alongside
home health/hospice aides. Candidates should be able to demonstrate
strong experience, particularly with Oasis documentation.
Additional duties may be assigned based on patient needs.
