Details - Home health and hospice setting with patient-to-patient home visits - Monday through Friday, 8:00 AM – 5:00 PM schedule - Holiday coverage required as needed - Uses HealthWyse EMR Requirements - Minimum 2 years of physical therapy experience required - OASIS experience required - Home health and hospice experience required - Must have experience traveling between patient homes for in-home visits - Active Vermont PT license required if applicable - Must be able to manage and adapt to schedule changes Additional Information - Responsibilities include evaluation and treatment of patients in a home health and hospice environment - Must be flexible and self-directed given the nature of home visit scheduling - Facility conducts its own background check American Traveler Job ID P-688265.
